Neurostymulacja (lub neuromodulacja) jest metodą terapeutyczną polegającą na sztucznym pobudzaniu układu nerwowego pacjenta impulsami elektrycznymi. Urządzenie elektroniczne realizujące tą funkcję zwane jest neurostymulatorem. Jednym z obszarów neurostymulacji jest neurostymulacja wegetatywna, to jest neurostymulacja wegetatywnego (autonomicznego) układu nerwowego. W tym obszarze szczególnym zainteresowaniem cieszy się stymulacja nerwu błędnego (ang. Vagus Nerve Stimulation, VNS). Przedmiotem pracy jest opracowanie wszczepialnego, adaptatywnego neurostymulatora przeznaczonego do badań nad terapeutycznymi zastosowaniami neurostymulacji wegetatywnej na modelach zwierzęcych. Nowością opracowanego rozwiązania jest wyposażenie neurostymulatora w specjalizowany układ ASIC przeznaczony do ciągłego monitorowania aktywności nerwowej, w celu umożliwienia pracy urządzenia w zamkniętej pętli sprzężenia zwrotnego, to jest generowania impulsów jedynie w sytuacji braku naturalnego sygnału sytości. Praca obejmuje analizę naturalnej sygnalizacji nerwu błędnego u szczurów oraz opracowanie technik filtracji i dyskryminacji sygnału w celu umożliwienia analizy statystycznej impulsacji nerwu błędnego szczura pod kątem sposobu kodowania naturalnego sygnału sytości. Wiedza ta jest następnie wykorzystywana do zaprojektowania układu ASIC do analizy aktywności nerwowej w czasie rzeczywistym. Ostatecznie układ ASIC zostaje zintegrowany jako element wszczepialnego, programowalnego i adaptatywnego neurostymulatora.
Neurostimulation (or neuromodulation) is a therapeutic method operating on the principle of artificial stimulation of the patient’s nervous system with electric pulses. An electronic device implementing this function is called a neurostimulator. One of the areas of neurostimulation is vegetative neurostimulation, i.e. neurostimulation of the vegetative (autonomous) nervous system. In this area of particular interest is vagus nerve stimulation (VNS). The subject of the thesis is development of an implantable, adaptive neurostimulator destined for experimentation into therapeutic applications of vegetative neurostimulation on animal models. A novelty of this system is equipping it with a specialized ASIC circuit for on-line monitoring of neural activity, in order to allow for closed-loop operation, i.e. generating pulses only in the absence of the natural satiety signal. The thesis includes analysis of natural vagus nerve signals in rats, and development of techniques for filtering and discriminating of signals in order to allow statistical analysis of vagus nerve signaling for discovering the natural method of coding of the satiety signal. This knowledge is then used to develop an ASIC circuit for real-time monitoring of neural activity. Finally the ASIC circuit is integrated as an element of an implantable, programmable and adaptive neurostimulator.
